Application for housing - military
You must be active duty with a Common Access Card (CAC) to be able to complete the forms with electronic signature. Complete Blocks 1 - 21 of the application, click block 22 to digitally sign. Your electronic signature and the date will be entered on the form. In addition to the above documents, the Military Housing Office (MHO) will require your PCS orders and a recent copy of your LES in order to verify eligibility. When complete, e-mail all requirements to MHO at 55CES.CEIH.OffuttHousing@us.af.mil.
